Our Services

CCI Support provides person‑centred and whānau‑centred support for people living with long‑term conditions (LTCs), including ME/CFS, Long COVID, fibromyalgia, dysautonomia, and other complex chronic conditions. Our services are designed to strengthen self‑management, improve wellbeing, and enhance health equity for people whose needs are not fully met by existing care pathways.

We walk alongside individuals and their whānau, offering practical tools, guidance, and connection to help them navigate their health journey with confidence.

Qualified Support Field Officers

Self-Management Support

Our Health & Wellness Facilitators provide one‑on‑one support to help people understand their condition, develop personalised strategies, and build confidence in managing daily life. This includes:

  • pacing and energy management
  • symptom management strategies
  • lifestyle and wellbeing guidance
  • goal setting and planning

Self‑management support is delivered in a compassionate and accessible way.

Towards Wellness Programme

Navigation & Advocacy

People living with long‑term conditions often face complex, fragmented care pathways. We help individuals:

  • understand diagnoses and medical information
  • prepare for appointments
  • navigate referrals and services
  • advocate for equitable access to care

Our role is to ensure people feel informed, supported, and empowered within the health system.

Friendly Group Seminars

Peer Support & Community Connection

Living with a long‑term condition can be isolating. Our peer support groups — both online and in‑person — provide a safe space to connect with others who understand the journey.

We offer:

  • facilitated peer groups
  • condition‑specific groups
  • community connection opportunities
  • shared learning and encouragement

These groups reduce isolation, strengthen resilience, and build a sense of belonging.

Whānau Support

Long‑term conditions affect the whole whānau. We provide support, education, and guidance for family members so they can better understand the condition and feel confident supporting their loved one.
